CORONARY ARTERY DISEASE

HeartIn a healthy human being, the heart muscle has an adequate blood supply to contract properly. When the coronary arteries (carrying oxygen) and blood supply to myocardium (heart muscle) gets narrowed or blocked, the area of the heart muscle supplied by that coronary artery suffers Ischemia (lack of blood supply), injury and infarction (death of myocardium). This is what is known as Ischemic Heart Disease or Coronary Artery Disease.

Coronary heart disease (CHD) is characterized by the accumulation of fatty deposits along the innermost layer of the coronary arteries. The fatty deposits may develop in childhood and continue to thicken and enlarge throughout the life span. This thickening, called Atherosclerosis, narrows the arteries and can decrease or block the flow of blood to the heart. Acute Coronary Syndrome, Angina Pectoris, Myocardial Infarction and Progressive CAD are the most common manifestations of Coronary Artery Disease or CAD.

Angina pectoris literally means "Pain in the chest" as a symptom of Myocardial (heart muscle) Ischemia.

It is a transient chest pain caused by Myocardial Ischemia lasting only few minutes (3 - 5 minutes) and commonly subsides when precipitating factors is relieved (usually exertion). This is called Exertional Angina.

 
Types
  • Stable or Classic Angina
    The pain in this case is predictable and caused by similar precipitating factors every time such as:
    • Exercise
    • Emotional or Psychological stress
    • Any activity that increases the myocardial oxygen demand
    • Rest and sublingual nitroglycerine relieves the pain

  • Unstable Angina
    The pattern of onset of pain is not predictable and classified as intermediate in severity between stable angina and Myocardial Infarction.

  • Myocardial Infarction
    Myocardial infarction is an irreversible cell death of the myocardium resulting from complete occlusion of coronary blood flow to the specific area of the myocardium (heart muscle).

  • Stable / Progressive CAD
    In majority of patients with angina, once the stable plaque (fat deposition) has dislodged from its place, it injures the inner blood layer causing clot formation in the blood vessels.

    This unstable clot is at increased risk of complete thrombosis (blocking the blood vessels) of the lumen leading to Myocardial infarction.

    This is why these patients require immediate hospitalization with ECG monitoring and bed rest.
