Coronary Bypass without anaesthesia...

For the first time in our country, a coronary bypass surgery has been conducted without anaesthesia or use of a heart lung machine, meaning the patient was fully conscious during the surgery. This clinical feat was performed by a Wockhardian - Dr. Vivek Jawali, Chief Cardiac Surgeon, and his team at our Bangalore-based Wockhardt Hospital & Heart Institute (WHHI).

How WHHI performed this pioneering surgery:

The operation was performed on a 78-year old man who had multiple complications including diabetes, renal problem and blockages in the carotid arteries supplying blood to the brain. The patient, a smoker, also had affected lungs.

On 17th January, the patient was rushed to a major hospital with chest pain. There, an emergency coronary angiogram indicated the urgent need for a coronary bypass surgery. However, due to his various complications, the patient could not be operated upon. His affected lungs would not be able to tolerate general anaesthesia and the multi-organ dysfunction made him unfit for the heart lung machine. The patient was rushed with his family to WHHI. Dr. Jawali and his team on examining him came up with a medical plan to perform a minimal invasive cororary bypass without general anaesthesia or ventilator support and without major sedatives or muscle relaxants. Dr. Jawali decided to use the technique of high epidural analgesia, a medicine that would numb the pain in the chest while keeping the patient fully conscious. This skillful operation lasted 45 minutes, and the patient was fit to be sent home after four days.

Needless to say, the patient and his family have been jubilant and the operation made front-page news in the city.
