 ADMISSION PROCEDURES - MUMBAI
 
admissionYour personal doctor or the consultant doctor at Wockhardt Hospital Mulund, has arranged for you to be admitted to the Wockhardt Hospitals for medical treatment. To begin the process, you or a member of your family will need to meet a member of the Costumer Care personnel to complete admission formalities.

At the reception, an Admission and Authorization Form will have to be filled in with the necessary information, to enable us to register you as a patient. The Authorization Form includes an approval for the hospital staff to provide medical care and treatment to the patient. All patients below the age of 18 years, require the authorization from a parent or guardian.

 
Rooms
Wockhardt Hospitals, Mulund offers a choice of Special Deluxe, Twin Deluxe and Multibed rooms. Every effort is made to provide the type of room that you want to occupy. However, at times, a particular type of room may not be available because all such rooms are either occupied or booked. In such an event, the hospital will allot you the best alternate room available. Request for the room change may be made to the front office at the extension 4133 who will attempt to meet your request as soon as it is possible.
 
Your personal belongings
Pack light when you come to the hospital for admission. You will only require a few belongings such as toiletries and slippers at the hospital.
 

Valuable
Please do not keep valuables in excess amounts of money with you. We advice you to ask relatives to look after your valuables and money as the hospital cannot accept responsibility for the loss or damage to your possessions.

 
Lockers
If you cannot arrange for your valuables to be kept for you, lockers have been provided in each Special Deluxe and Twin Deluxe room for your convenience. A key to the locker will be handed over to you along with your room keys at the time of admission. Please check the locker as soon as you get admitted to the room. The Hospital however cannot accept responsibility for money or valuables kept in the lockers.
